Primary Care Worker

Salary: NJC Pt 9 -19 (£21,269 - £25,927) depending on experience.

Hours: 37.5 Hours (5 Days)

Location: Bath and surrounding areas, including GP surgeries

Contract Type: Permanent

The Role

DHI are currently recruiting for a full time worker to support individuals experiencing problematic drug use. We are looking for inspiring and creative individuals who are passionate about providing a person centered approach towards support for people who are often marginalized from society.

As part of the Shared Care Team you will manage a caseload of service users and will work in close collaboration with GPs, partner agencies and other health and social care professionals. Internal training will be provided.

You will have the opportunity to work flexibly between home, DHI offices and GP surgeries to support clients.
