Salary: NJC Point 18-20 (£24,313-£25,295) pro rata

Hours: Up to 30 hours

Contract Type: 2 year Fixed Term Contract

The Role

This year, do you want to Help People Turn their Lives Around by enabling them to improve their health, wellbeing and resilience? If so, Developing Health & Independence (DHI) and West of England Rural Network’s (WERN) social prescribing project in Bath and North East Somerset could be the right job for you.

We are working with Virgin Care and local GPs to provide a new and innovative Social Prescribing Service which will enable people to change their lives by connecting them into community services, groups, activities and other pursuits.

As a social prescriber you will work in a personalised way with a range of people who have different levels of needs. You will develop and maintain and excellent knowledge of what’s out there in the local community and will supervise volunteers.

If you have at least 2 years of working with people who have health or social care needs we would love from you.
